How Information is Used: Information collected from this page is used to match the web user with physicians on staff at Baptist who meet the criteria supplied by the user and physicians. The user is also added to our mailing list and may receive direct mail notices of special events, such as health screenings. The user may also receive a quarterly newsletter.

See a Surgeon at Baptist
The purpose of this web page is to assist our web users in locating a surgeon based on the user’s expressed needs and interests.
Information Collected: On this page, a user provides his/her full name; e-mail address; gender; complete mailing address; day time and evening telephone numbers; date of birth; health insurance information, including whether or not he/she has Medicaid or Medicare; symptoms or reason for requesting a surgical referral; selection of a surgical speciality. Register Online
The purpose of this web page is to allow patients to complete and submit much of the registration paperwork for an upcoming inpatient or outpatient visit over the Internet.
Information Collected: On this page, a user provides his/her full name; Social Security number; date of birth; marital status; sex; race; complete mailing address; e-mail address; home telephone number; church; denomination; employment information, including occupation or retirement date, employer’s name, telephone number and address; date and reason for visit to Baptist; emergency contact information including full name, address, home telephone number, relationship to patient, employer’s name, address and telephone number, occupation and retirement date; guarantor’s full name, Social Security number, date of birth, marital status, sex, race, complete mailing address, home telephone number, occupation, date of retirement, and employer’s name, address and telephone number; primary and secondary insurance information including insurers’ name, address for filing claims, telephone number, the policy holder’s name and relationship to the user, policy holder’s date of birth, policy ID number, group name and group number.
How Information is Used: All information submitted in the online pre-registration process becomes part of the user’s confidential medical record. All the laws and policies that govern medical records apply. Please read the Notice of Privacy Practices.

Baptist Care Mail
The purpose of this page is to allow web users to send messages to inpatients via the Internet.
Information Collected: On this page, a user provides the patient’s name and room number; the user’s name, address, telephone number and e-mail address; a message to the patient.
How Information is Used: Messages are printed and delivered to the patient designated by the web user. If more information is necessary to identify the patient receiving the message, the user will be sent an e-mail message to request it. If the patient has been discharged, the messages are forwarded to the patient’s home address. No information from this page is retained for any further purpose.

Contact Medical Staff Services
The purpose of this page is to allow web users to contact members of Medical Staff Services over the Internet.
Information Collected: On this page, a user provides his/her name, telephone number; e-mail address; and message.
How Information is Used: Staff in Medical Staff Services respond to the user’s message. No information from this page is retained for any further purpose.

Ask a Certified Diabetes Educator
The purpose of this page is to allow web users to ask a question about diabetes.
Information Collected: On this page, a user provides his/her name; sex; complete address; e-mail address; daytime phone; evening phone; date of birth; and may include Social Security number. The user also answers a series of questions related to his/her diabetes, and then supplies a specific question. The user also indicates a preference regarding receiving mailings from Baptist Health Systems.
How Information is Used: Information collected from this page is used to give diabetes educators background to help them more appropriatey answer the user's question. Information related to the person's diabetes experience is confidential and not made part of any permanent record or retained for any purpose. Users who request to do so are added to Baptist's mailing list.

The purpose of this page is to allow businesses to contact Corporate Health Services online. Users are asked to provide the company's name, company contact, title, address, phone, fax and e-mail and information indicating an area of interest.  
How Information is Used: Information submitted is used to respond to users' requests via e-mail or mail. No other information from this page is retained for any further purpose.

Online Job Application
The purpose of this page is to allow users to submit a job application online.
Information Collected: Users are asked to provide employment information such as the relevant position, shift preference, desired income, days/hours available if part time, full name, address, day, evening and alternative telephone numbers, e-mail address, Social Security Number, License, Registration or Certification number, information about personal transportation and any previous terminations. Users also provide information about education, employment record and personal references.
How Information is Used: Online job applications are retained in our files for 90 days. Human Resources will review applications for consideration of employment. Applicants selected for personal interviews are notified by telephone. No other information from this page is retained for any further purpose.

Become a Patient at Baptist Cancer Services
The purpose of this web page is to assist our web users who wish to become patients at Baptist Cancer Services.

The purpose of this web page is to assist physicians who wish to refer patients to Baptist Cancer Services.
Information Collected: On this page, a physician provides his/her full name; complete address; e-mail address; day time telephone numbers; patient's full name; gender; complete address; e-mail address (optional); daytime and evening telephone numbers; date of birth; Social Security number (optional); diagnosis; date of diagnosis; reason for referral; pertinent medical history; name, title and daytime telephone number and extension for an office contact.
How Information is Used: Information collected from this page is used to facilitate the physician's referral of a patient to Baptist. Your Health-E Life
The purpose of this web program is to assist our web users who wish to store and manage personal health information online via Baptist's website and to interact with Baptist online regarding personal health information.
Information Collected: For this program, a user provides his full name; e-mail address; Social Security number; and gender. The user also chooses a user name and password.
How Information is Used: The user's personal health information stored online is in a secure area and accessible by a user-defined name and password. Baptist makes no attempt to obtain or use this information for any reason without the user's consent. (For example, Baptist will, at the user's request, access the user's account to reset a password).
The user is added to our mailing list and may receive direct mail notices of special events, such as health screenings. The user may also receive a quarterly newsletter.

Special Programs
From time to time Baptist Health Systems may provide users an opportunity to submit information online in connection with a special event or program, such as the Strong Women Awards. The purpose of these online forms is to provide a convenient means for users to interact with Baptist Health Systems.
