A Complete Program Dedicated to Helping People With Diabetes Lead a Healthy Life
Successful management of diabetes depends primarily on individuals taking responsibility and control of their own health. Baptist Health Systems offers a Diabetes Self-Management Education Program recognized by the American Diabetes Association for its quality of care. The program has also achieved Disease-Specific Care Certification from the Joint Commission (JC).

This program is uniquely designed to provide our patients with a comprehensive approach to diabetes. We provide a series of planned learning experiences to enhance your knowledge of basic skills such as meal planning, medication administration, self blood glucose monitoring and exercise. We will assist you in developing individualized plans and goals to meet your lifestyle needs.

ADA Recognized Diabetes Self-Management Education Program
At Baptist, we provide a multi-disciplinary approach in formulating the best plan for diabetes management and for minimizing complications. This program is not intended to replace your primary care physician, but is designed to meet the individual educational needs of a person with diabetes.

Team members include only qualified personnel:

  • Certified Diabetes Educators
  • Registered Dietitians
  • Registered Nurses

Program Content
We provide group classes or one-on-one educational sessions and encourage family members to attend. The Diabetes Self-Management Education Program also offers a follow-up session specific for the patient to evaluate his/her skills and goals. You will learn about:

  • Diabetes overview
  • Management of medication and exercise
  • Medical nutrition therapy
  • Detection, prevention and treatment of diabetic complications
  • Self glucose monitoring
  • Sick day guidelines
  • Insulin administration

We place an emphasis on providing a personal touch for our patients with diabetes. We also have other opportunities and resources available for our patients with other special needs. Some of these services are:

  • Insulin Pump Therapy with hands-on training for patients (inpatient or outpatient)
  • Gestational diabetes (diabetes in pregnancy)
  • Diabetic Support Groups

The Baptist Diabetes Learning Center is located in the Medical Arts West Building, on the 4th floor in Suite 410.
